Information on Collecting Medical Reports and Clinical Records

Reports from medical visits are delivered directly by the doctor at the end of the visit.
For other types of services (such as laboratory tests and instrumental exams), unless otherwise specified, reports will be available through the methods described below.

Through the portal www.istitutotumori.mi.it, using MyPage via this personal section, it is possible to access some services of the IRCCS National Cancer Institute Foundation online. Specifically, you can:

  • View and download your own reports (except for Anatomical Pathology reports) and diagnostic images from the last 45 days, as required by regulations (Guidelines on online reports – November 19, 2009, by the Data Protection Authority)
  • View administrative documentation related to services performed
  • Fill in a reserved notes section (Notebook)

At first access, informed consent will be acquired (which can be revoked at any time), allowing you to use these online services.
This service is reserved for adult patients.

For residents of Lombardy, it is possible to view your report online using SPID.
Additionally, by consulting the report online, patients are exempt from the penalty fee normally applied if the report is not collected within 30 days from the date indicated on the collection slip.

The reports available online include:

  • Specialist Visit Report
  • Laboratory Report (excluding Pap tests, histological and cytological exams, and prenatal diagnostic tests)
  • Radiology Report
  • Hospital Discharge Letter

For patients who do not reside in Lombardy, it is possible to receive medical reports (with the exception of genetic reports) via email.

The request must be made to the referring specialist at the Institute, who will record your specific consent in the IT system and ask you for the email address where the report should be sent.

In compliance with data protection regulations, and to prevent your data from being accessed or read by unauthorized individuals, the report will be sent in encrypted format (not directly readable), and the password to open the file will be communicated to you by phone.

Using the self-service Totems located in the Reception Area, in the main lobby of the Foundation.

At the Totems, it is possible to collect printed reports for Laboratory tests and Diagnostic Imaging exams.
To use this service, you must have your Health Insurance Card (Tessera Sanitaria) and either the acceptance receipt or the appointment reminder, and follow the on-screen instructions.

For CD printing of imaging exams, please go to the Outpatient Desk, after collecting a ticket number from the queue system.

The Totems for report printing are available from Monday to Friday, 8:00 AM to 4:00 PM.

To collect your reports, you must present your Health Insurance Card and the receipt of service acceptance.

If someone else is collecting the reports on your behalf, they must present a written authorization, using the Authorization Form for Collecting Copies of Clinical Records and Reports.
The authorized person must provide:

  • The completed authorization form
  • A copy of the ID of the person granting authorization
  • Their own ID

To request shipment of reports or images on CD, you must fill out the Request Form for Reports or Images on CD.

The Report Collection Desk is located in the General Outpatient Clinic on the ground floor, and is open from Monday to Friday, 8:00 AM to 4:00 PM (no break).
You must take a ticket (queue 12) from the queue ticket machine at the desk.
