The Department of Anesthesiology and Intensive Care is responsible for the anesthetic management of patients undergoing surgical procedures within the Foundation’s operative units (7,000–8,000 surgeries per year), as well as for their perioperative care.
The Intensive Care Unit (Director: Dr. Daniela Codazzi) provides six beds for the most complex postoperative cases, critically ill non-surgical patients (500–600 admissions per year), and the management of critically ill patients in inpatient wards.
The team also ensures anesthetic support for diagnostic and therapeutic procedures, particularly in pediatric patients, carried out in the departments of radiology, radiotherapy, nuclear medicine, endoscopy, and in the Institute’s day hospital.
Other cross-cutting commitments of the department’s staff (physicians and nurses) are:
- placement and management of central venous catheters, including long-term systems, which are required for patients undergoing long-duration therapies.
- the “acute pain service”, which follows operated patients in the first 3–5 days in order to optimize the treatment of postoperative pain.
This activity has as its main backbone the resident physicians of the School of Anesthesiology and Intensive Care of the University of Milan, who attend the Service for periods from 6 months to 2 years for training in Anesthesia.
The supervision of this activity is entrusted to an experienced nurse and to some staff physicians, in collaboration with physicians of the Pain Therapy.
The Service fulfills two important functions: the improvement of postoperative pain treatment and the training of young physicians in attention to pain. - emergency–urgency in the Hospital: in addition to constant availability for direct intervention, Dr. Codazzi and her collaborators ensure training and coordination at the various levels necessary for all the operators of the Institute.
Scientific research activity is mainly directed toward topics of anesthesia and pain therapy. Collaborations with other working groups in Italy and abroad are ongoing.
The clinical activity of the Anesthesiology and Intensive Care Service focuses on:
- Preoperative assessment in wards and in the dedicated outpatient clinic
- General and loco-regional anesthesia for patients undergoing surgery
- Sedation and analgesia for day-surgery patients
- Postoperative pain management using various techniques (elastomeric pumps, patient-controlled analgesia, epidural analgesia), coordinated by the Acute Pain Team
- Anesthesiology support for invasive procedures outside operating rooms
- Sedation/anesthesia for pediatric patients undergoing painful procedures
- Placement of central venous catheters (temporary or fully/partially implanted) in adults and children
- Consultation and management of critically ill patients, including emergency interventions in hospital wards (Intensive Care Unit)
- Organization and coordination in case of major hospital emergencies (Intensive Care Unit)
- Training courses for hospital staff in BLS-D, pain therapy, and vascular access management

“Improvement of acute and chronic pain management at the ‘Fondazione IRCCS Istituto Nazionale dei Tumori’ of Milan; research on the organization and implementation:
a) institution of an ‘acute, postoperative and procedure-related pain team’;
b) implementation of invasive procedures such as neural blockade or implementation of specialized devices for drug delivery in patients with severe pain difficult to control with systemic drug administration;
c) implementation of chronic pain and symptom control: multidisciplinary hospital team”
funded by the Italian Ministry of Health (5‰ / year 2005) (Principal Investigator: Prof. M. Langer).

Collaboration in the GiViTI project.
“Margherita – Prosafe”, Network of Italian intensive care units (Intensive Care Units) (Coordinators: Dr. Valerio Costagli, Dr. Myriam Favaro, Prof. Martin Langer).
Educational and training activities are mainly focused on postgraduate education (Residency School in Anesthesiology and Intensive Care), with the continuous presence of 4–7 residents.
Residents are typically assigned to the unit for 1 to 1.5 years, during which they are involved in all clinical and training activities, with responsibilities tailored to their level of experience. Many prepare their final thesis based on clinical and research work conducted at the Institute.
Formal teaching for second-year residents and common seminars of the School are held at the Foundation.
In addition, several physicians from the unit are involved in teaching within the Nursing Degree Program, other specialty schools (Oncology, Surgery), and participate as speakers in national and international courses and conferences.
