The Intensive Care Unit (ICU) is part of the SC of Anesthesia and Intensive Care Unit, within the Department of Oncological Surgery. Located on the first floor of Block F, the ICU operates 24 hours a day, 7 days a week, with a clearly defined mission: to provide highly specialized care to critically ill oncology patients, wheter their condition is medical or surgical, affected by one or more potentially reversible acute organ failures.  
The Unit is organized as an “open” Intensive Care Unit, promoting the presence of family members and interdisciplinary collaboration throughout the care process. Visitor access is allowed every day from 1:00 p.m. to 6:30 p.m. , with one person at a time per patient and in compliance with the planned protection measures. Information to relatives is provided every day around 2:00 p.m. by the attending physician.  
 

Clinical and Care Activities 
In addition to direct assistance within the ICU, the Structure is strongly integrated with the entire hospital system, helping to ensure continuity and quality of care even outside the department:  
- 24-hour in-hospital emergency management, through a team composed of a Critical Care Consultant and a nurse. 
- Critical Care Consultations and counseling services to other departments. 
- CAR-T counselling for preevaluation of eligible patients. 
- Protected secondary patient transport , available 24/7 with specialized teams. 
- Support for the Acute Pain Service (APS), with nursing staff trained in the management of moderate-severe postoperative pain in patients undergoing major surgery. 
- N.O.R.A. endoscopic activities: sedation for NORA procedures in collaboration with the Gastroenterology and Digestive Endoscopy Unit. The procedures involved include ERCP, endoscopic ultrasound, cholangioscopy, and other advanced diagnostic-therapeutic techniques. 

Teaching and Training 
The ICU plays an active role in University and Residents training: 
- site of annual assignment of two residents 
- hosts Medical and Nurse Students for clinical internships 
-organizes and conducts training courses on the management of intrahospital emergencies and acute postoperative pain, also through residential courses and post-event debriefings.  
 

The Intensive Care Unit is actively involved in scientific activities, contributing to the development of clinical research in the field of oncological intensive care, in collaboration with academic institutions and national research centers. Its research activities include multicenter projects, participation in study groups and the development of innovative protocols, with particular attention to the continuous improvement of care quality for frail patients.  

The ICU promotes an integrated and multidisciplinary care model, based on operational synergies with numerous internal structures of the Hospital and with nationally relevant external partners. These collaborations are aimed at clinical innovation, applied research and the continuous improvement of care quality, especially for critically ill oncology patients.  


Internal Clinical Collaborations: 
- Gastroenterology and Digestive Endoscopy Unit: established cooperation for sedation activities in N.O.R.A. procedures during complex endoscopic procedures. 
- Medical Oncology Unit: participation in drafting the company PDTA for the management of oncology patients with tromboembolic complications. 
- Anesthesia and Operating Block Unit: collaboration in managing the Acute Pain Service. 
- ICT Service: active in a working group for the digitization of discharge letters and digital archiving of hospital stays. 

Collaborations with Research Centers and Universities: 
- Polytechnic University of Milan 
- Mario Negri Institute – GIVITI: participation in the PROSAFE study and in the development of the “Margherita 3” clinical record. 
-University of Milan: accredited training site.
